CSAR User Survey 2005

This is the seventh annual user survey to be carried out by CSAR, covering the period from January to December 2005. The purpose of the survey is to obtain an accurate picture of how the service is viewed, to identify any weaknesses in service provision and to help determine future needs and priorities.

We hope that you will find the time to contribute to the survey to help us improve the service. The form has been designed to gather views on a variety of topics and has ample space for comment - please answer as many of the fourteen questions as you wish.

This year we are holding a prize draw. The winner will be picked at random from those who submit the User Survey form and will receive a bottle of malt whisky (or agreed alternative).

Your response is requested by 16th December 2005.

Please ensure you click the submit button at the end of the form when you have finished. (Being an html form, your entries cannot be saved for later retrieval and submission. You may wish to print a copy for your own records.)

Submitted forms will be entirely anonymous, unless you wish to give us your name at the end. Please remember that if you have submitted your form anonymously we will not be able to enter you into the prize draw. In any case, answers will be treated in the strictest confidence and a summary of the results will be included in the Reports section on the CSAR web pages.
